29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification:   a)  Outside Area, Back Pad Area, on or about 9/13/16:  The Dry Finisher was exposed to airborne respirable silica at a concentration of .690 mg/m3 time weighted average, 1.68 times the permissible exposure limit (PEL) of .41 mg/m3.  The PEL was based on 22% crystalline silica in the sample. The sample was collected on 9/13/16 for 223 minutes. Zero exposure was assumed for the 257 minutes not sampled. Employee was not wearing a one strap dust mask respirator that was not NIOSH approved.

The sampling period was for 122 minutes. A zero increment is included for the 358 minutes not sampled. Employee was wearing a N95 Dust Mask Respirator.    b) Outside Area, Back Pad Area, on or about 9/13/16, a dry finisher grinding concrete was exposed to .690 milligrams per cubic meter (mg/m3) of airborne silica for a eight hour time weighted average (TWA) which was approximately 1.68 times the OSHA permissible exposure limit of .410 mg/m3. The sampling period was for 223 minutes. A zero increment is included for the 197 minutes not sampled. Employee was wearing a one-strap Dust Mask Respirator.
