At the concrete mixer, on or about 8/16/18: Employees were entering the concrete mixer, designated as a permit required confined space, to perform maintenance such as but not limited to cleaning and paddle adjustment. Atmospheric testing was not performed prior to the employees entering the permit space. Abatement certification is required for this item.
