29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about June 20, 2016, employees were working below other employees installing cornice materials on the exterior of the residential home under construction without wearing head protection, exposing the employees to the hazard of being struck by falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about June 20, 2016, two employees were installing cornice materials on the residential home under construction up to approximately 18 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about June 20, 2016, employees were installing cornice materials on the residential home under construction on job-made scaffolding up to approximately 16 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    On or about June 20, 2016, employees were using a 6 foot, portable, free standing ladder leaned against the exterior of the home under construction and resting on a 12-inch scaffold platform to access the second floor, exposing the employees to a fall hazard up to approximately 16 feet high.
