29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):  On or about April 11, 2016, the employer did not ensure that tanks on the B-Line to include but not limited too etch tanks, nitric acid tanks, sulfuric acid tanks, hydrochloric acid tanks, and copper acid tanks were labeled with the identity of the chemical and the appropriate hazard warning label.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  On April 11, 2016, the employer did not report the in-patient hospitalization of an employee, who sustained chemical burns to OSHA with in 24 hours. The incident occurred on March 14, 2016 at 9:00pm.
