29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about May 26, 2016; Roof area of residential home (Eastside); employees were not using fall protection to protect from falling an estimated 35 feet to ground level while installing the shingles .                                                                         b) On or about May 26, 2016; Roof area of residential home (Westside); employee was wearing rock climbing equipment as a means of fall protection exposing him to an estimated ed 35 feet to ground level while installing the shingles.

29 CFR 1926.502(d)(5): Non-locking type snaphooks were used as part of a personal fall arrest system:   a) On or about May 26, 2016; Roof area of residential home (Westside and Eastside); the personal fall arrest system used in the process did not have a  snap hook system in place to hook-up to the anchor point exposing them to a 35-foot fall to ground level while working on 12:12 pitch roof; rope was tied off to the anchor point.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:    a) On or about May 26, 2016; eastside of building; employees did not maintain a three point of contact while carrying bundles of shingles to the roof area via the ladder; exposing them to a potential estimated fall of 35-feet.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
