29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Worksite: The full body harness, worn by an employee working from the basket of an aerial lift was not attached to the basket or boom with a lanyard. On or about 06/22/2016, the aerial lift was hit by a heavy truck while the employee was working from the elevated basket, causing the employee to fall out from the basket and landed on the ground. The employee sustained serious injuries from the fall.
