29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. Throughout facility - On or about June 23, 2016, employees were required to service and maintain equipment as part of their regularly assigned duties. The employer did not effectively document periodic inspections of Lockout / Tagout procedures in accordance with the aforementioned standard. Employees utilizing the procedures were thereby exposed to the hazards associated with unexpected re-energization of equipment.
