Condition(s) were noted or about 6/24/2016. **Note** Does not apply to employees who have previously received the complete Hepatitis B vaccination series, for whom antibody testing has indicated immunity, or for whom the vaccine is contraindicated for medical reasons. This employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1084403, citation number 1 item number 2a and was affirmed as a final order on 12/19/2015, with respect to a workplace located at 1814 E. Second St., Scotch Plains, NJ 07076.
