29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about 21 June 2016 and at times prior thereto, four employees were performing framing operations on a residence under construction without head protection, exposing the employees to the hazard of being struck by falling or flying objects.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 21 June 2016 and at times prior thereto, four employees were performing framing operations on a residence under construction without eye protection, exposing the employees to the hazard of being struck by falling or flying objects.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe.  On or about 21 June 2016 and at times prior thereto, two employees were cutting framing materials using a circular saw with the guard blocked open by a wooden wedge, exposing the employees to the hazard of being struck by the exposed blade of the saw.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 21 June 2016 and at times prior thereto, two employees were performing framing operations on the second floor of a residence under construction without fall protection, exposing the employees to the hazard of falling approximately 9 feet to the ground below.
