29 CFR 1910.178(p)(1):     A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck and was not  be taken out of service until it had been restored to safe operating condition.    On or about 6/27/2016, employees working at Washington Lumber, located at 620 Market Street, Washington, Missouri, were operating powered industrial trucks in need of repair, defective and unsafe.  The following defective forklifts were unsafe and being operated on site:    a) Caterpillar Forklift; serial # 12-02197, had a damaged seat and wood sheathing tied with rope to the roof, which restricted the operators view.  b) Caterpillar Forklift; Serial # AT13D-55016, had a damaged seat and hazy Plexiglas attached with a tie-down strap to the roof, which restricted the operators view.  c) Caterpillar PIT; Serial # 77X1827, had a damaged seat and wood sheathing screwed to the roof, which restricted the operators view.    Washington Lumber was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1), which was contained in OSHA inspection number 965118, citation number 1, item number 1-b and was affirmed as a final order on 8/28/2014, with respect to a workplace located at 620 Market Street, Washington, Missouri.
