29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. On or about 6/27/2016, employees working at Marthasville Lumber, located at 104 Commercial Drive, Marthasville, Missouri, were exposed to fall hazards. Standard railing or guardrails were missing in the following areas: a. Main Building bay #1; parts of the mezzanine, which was approximately nine feet above the floor, were missing guardrails or standard railings. b. Upper Building; no guardrails or standard rails were installed on the mezzanine, which was approximately nine feet above the floor.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails. On or about 6/27/2016, employees working at Marthasville Lumber, located at 104 Commercial Drive, Marthasville, Missouri, were exposed to fall hazards. The stairway for the mezzanine in the upper building did not have standard stair railings or standard handrails installed.
