29 CFR 1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted: On or about June 29, 2016, and at times prior to, employees were exposed to being struck-by soil from underneath the trench shoring/sheeting were set more than 2 feet above the trench floor, while employees installed conduit and copper lines inside the trench.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 29, 2016, employees were exposed to a fall while utilized a ladder, that extended approximately 1 1/2 feet beyond the landing, to access and egress from trench.
