29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems: a) 777 Hollywood Blvd, Grantville, PA - Employees were observed performing roofing activity on a 4/12 pitch roof without a conventional fall protection or an alternative system in place, thereby exposing employees to a fall of 11 feet to the ground below, on or about June 23, 2016. No abatement certification or documentation required. Pine Creek Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 315799668, citation number 1, item number 1 and was affirmed as a final order on February 28, 2012, with respect to a workplace located at 135 Tremont Road, Pine Grove, PA 17963.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a) 777 Hollywood Blvd, Grantville, PA - Employees performing work from an elevated roof location 11 feet above the ground below were not provided a ladder to access or descend the roof. Employees were using the truss framework and wall girt boards to climb up and down from the gable end of the building, on or about June 23, 2016. No abatement certification or documentation required.
