JLG Scissor Lift - On or about October 19, 2023, employees were observed climbing the guardrail system of the scissor lift (mobile scaffold) while at heights greater than 15 feet above grade. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a. JLG Scissor Lift - On or about October 19, 2023, employees were raising a scissor lift to approximately 20 feet above grade and then climbing the rail system to gain access to the trusses where they were walking the trusses to install supports. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Trusses - On or about October 19, 2023, employees were observed working between the trusses greater than 20 feet above grade without fall protection. The employees were bracing the trusses by climbing between the trusses with 2x4's to nail into place to serve as bracing. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
