29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:   a) On or about June 15, 2016 and at times prior thereto; second floor area; employees were plugged into an electrical box that was not installed permanently as it was intended.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On or about June 15, 2016; Second floor; employees working on framing the residential home were not protected from falling 10 foot fall to the lower level or 18 feet to ground level while working on the exposed working platform.     b) On or about June 15, 2016; Second floor; first and second floor; employees working on framing a residential home were exposed to a 10 foot fall to the lower level while working near unguarded floor openings such as but not limited to the stair opening.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed   a) On or about June 15, 2016; first floor; employees were using an extension ladder that was flush to the working platform having the employees to reach for the ladder or platform; 10-foot fall.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
