29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a. On or about June 29, 2016, in a Model SD64 Nifty aerial lift, an employee performing tree trimming operations was not protected from an approximated 45-foot fall hazard, in that the employee was not wearing a personal fall arrest system, resulting in him being ejected during operations from the basket to the surface below.
