29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   On July 29, 2016, Five Star Commercial Roofing was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:              Citation 2 Item 1.  The Five Star Commercial Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 980144, citation number 03, item number 001 and was affirmed as a final order on December 23, 2014, with respect to a workplace located at 2726 W Lake Avenue, Peoria, IL.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:    On or about June 29 2016, employees were exposed to a fall hazard approximately 20 feet while engaged in roofing activities and a suitable fall protection system was not used.    Five Star Roofing Systems, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 960851, citation number 1, item number 1 and was affirmed as a final order on May 23, 2014, with respect to a workplace located at 6315 Fauber Road, Bartonville Illinois, 61607.
