Ave, Copley Ohio 44321; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about April 3, 2023, during the installation of roofing material at a job site located at 85 Montrose W. Ave, Copley Ohio 44321; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 20.7 feet by utilizing a fall protection system. Five Star Commercial Roofing INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1442861, citation number 1, item number 1 and was affirmed as a final order on (6/17/2021), with respect to a workplace located at 1122 Columbus Ave, Washington Court House Ohio 43160.
