On or about April 3, 2023, during the installation of roofing material at a job site located at 85 Montrose W. Ave, Copley Ohio 44321; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 20.7 feet by utilizing a fall protection system.  Five Star Commercial Roofing INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1442861, citation number 1, item number 1 and was affirmed as a final order on (6/17/2021), with respect to a workplace located at 1122 Columbus Ave, Washington Court House Ohio 43160.
