29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On or about 08/28/2016, the employer failed to certify to OSHA that the following hazards have been abated:  (a)Citation 1, Item 1 - On or about June 27, 2016:  An employee installing metal studding on a new commercial construction project was exposed to fall hazards of approximately 15.4 feet without the use of fall protection measures while working from the basket of the aerial lift.  (b) Citation 1, Item 2 - On or about June 27, 2016:  Employee was exposed to fall hazards while working from an aerial lift when the employer failed to ensure that each employee was properly trained in the ability to reduce or eliminate the fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  On or about June 27, 2016:  a) An employee installing metal studding on a new commercial construction project was exposed to fall hazards of approximately 15.4 feet without the use of fall protection measures while working from the basket of the aerial lift.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 27, 2016:  a) Employee was exposed to fall hazards while working from an aerial lift when the employer failed to ensure that each employee was properly trained in the ability to reduce or eliminate the fall hazard.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
