29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, and flying chips.    On 11/2/16, in the Picking Room at the Athens facility, the employer did not ensure machine guarding provided was effective at the following machines:    (a) On or about 6/15/16, in the Picking Room at the Athens facility, employees manually removing paws from shackles left from the automated removal process were exposed to the hazard of being caught in a shackle without adequate means to stop the shackle line.    (b) On 11/2/16, Lines 1 and 2 in the Picking Room at the Athens facility, the doors of the auto rehanger machines, equipped by the manufacturer, were observed left in an open position.

29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.    On 11/2/16, in the Picking Room of the Athens facility, the employer did not guard the ventilation fan blades adequately on a fan that was within 3 feet of the floor. Louvers on the fan were 4 1/2 inches wide and when open did not guard the rotating blades.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).    On 7/19/16, in the Picking Power Room at the Athens facility, employees were exposed to 480 volt electrical hazards as employees, both sanitation and maintenance, routinely accessed electrical panels to turn breakers off for other equipment within the panel that was to be serviced and maintained.

On 7/19/2016, at the Athens facility, the employer did not have arc flash personal protective gloves inspected. Gloves were purchased in June of 2015 but have not been inspected since their initial purchase.
