29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator had been involved in an accident or near-miss incident: a) Palletizing Bay in the Dry Goods Warehouse- On or about June 20, 2016, the employer did not ensure that following a near miss incident involving a powered industrial vehicle, refresher training and an evaluation of the forklift operator's knowledge and skill were performed on relevant topics. New Albertson's, Inc. dba Jewel Osco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(ii)(B), which was contained in OSHA inspection number 1006712, Citation number 1, Item number 3 and was affirmed as a final order on November 27, 2015, with respect to the workplace located at 2200 N. 17th Avenue in Melrose Park, Illinois 60160.
