29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a) 700 West Lancaster Avenue, Wayne, PA - Warehouse: The employer did not ensure that each powered industrial truck operator was competent to operate powered industrial trucks and electric pallet jacks such as, but not limited to Hyster Type E Industrial Truck, exposing employees to crush/struck-by hazards. Condition noted on or about May 31, 2023. Training shall consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.
