Analytical Services, Inc. - On or about 7/6/16, and at times prior thereto, employees were exposed to fire hazards and fall hazards form the lack of emergency illumination.  The passageway/exit routes throughout the lab were not equipped with devices to provide adequate lighting during a power outage.

Analytical Services, Inc. - On or about 7/6/16, and at times prior thereto, the location of the ABC fire extinguishers were not marked.        B.)  Analytical Services, Inc. -  On or about, 7/6/16, and at times prior thereto,  some household fire extinguishers were not mounted but instead were set on tables.

Common fire hazards were present.  Fire hazards included, but were not limited to, common combustibles/paper products, and energized electrical equipment.

Analytical Services, Inc.; 1st floor - On or about 7/6/16, and at times prior thereto, fire extinguishers had not received an annual maintenance check.    The ABC fire extinguishers had not had an annual check since August 2013. Fire extinguishers without an annual maintenance check were located in areas that included, but were not limited to, the laboratory.

Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.  The employer shall provide the education required in paragraph (g)(1) of this section upon initial employment and at least annually thereafter:    A.)  Analytical Services, Inc. - On or about 7/6/16, and at times prior thereto, employees had not received initial and annual fire extinguisher training. Fire extinguishers including but not limited to Type ABC extinguishers were distributed in the workplace for use by employees.

The electrical outlets were not listed and labeled for use in a laboratory hood, hazardous locations, and/or contact with spilled chemicals.  The employees work with unknown flammables and corrosives in the laboratory hood.
