29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about July 6, 2016, the employer did not ensure that the machine specific procedure for locking out/tagging out the Centrifuge, NW Process Area, outlined all the steps for locking out/tagging out the equipment. a) The machine specific procedure does not indicate that tank is water flushed prior to opening. b) Step 8 indicates a line break for Nitrogen feed lines, which does not exist. c) Step 9 indicates that the centrifuge is ventilated, but lacks details on ventilation flow rate and time.
