The P&ID for the anhydrous ammonia system was inaccurate due to: a) P&ID AC04 (Rev. 02) represented a connection to a portion of equipment near compressor AC-100, which had been partially disconnected from the main system. The P&ID inaccurately represented a connections that had physically been disconnected downstream of a tee junction between valves GV-128, GV-132 and GV-160 and upstream of PRV-300.
