29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   (a)  On or about July 6, 2016, at a worksite located at 116 Tallmadge Circle, Tallmadge, Ohio 44278, during construction activities including, but not limited to roofing activities, employees were exposed to eye hazards while using pneumatic nailers.   (b)  O or about Jul 6, 2016, at a worksite located at 116 Tallmadge Circle, Tallmadge, Ohio 44278, during construction activities including, but not limited to roofing activities, employees were exposed to fall hazards of approximately 15-18 feet while installing roofing material.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about July 6, 2016, at a worksite located at 116 Tallmadge Circle, Tallmadge, OH 44278, during construction activities including but not limited to roofing activities, employees were exposed to fall hazards approximatley 15-18 feet while installing roofing material.
