On or about August 10, 2016 during the installation of roofing material at the job site located at 1225 Weber Ave Sw, Strasburg, Ohio 44680; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material utilizing air driven nail guns.
