29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    a) At 3900 Avenue G, N.W., Winter Haven, FL 33880, as observed on or about 07/20/2016:    1) For employees at the wall building line exposed to a continuous noise level at 223 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 95.78 dBA.     2) For employees operating a radial arm saw exposed to a continuous noise level at 256 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 96.78 dBA.     3) For employees at the floor truss building line exposed to a continuous noise level at 118 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 91.19 dBA.    4) For employees at the roof truss building line exposed to a continuous noise level at 67.2 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 87.13 dBA.    5) For employees feeding the auto web saw with carousel exposed to a continuous noise level at 77.1 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 88.12 dBA.     6) For employees receiving at the auto mill saw exposed to a continuous noise level at 90.8 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 89.30 dBA.    7) For employees feeding the auto web saw exposed to a continuous noise level at 129.6 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 91.87 dBA.    8) For employees feeding the auto mill saw exposed to a continuous noise level at 77.3 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 88.14 dBA.
