Lockout/tagout specific procedures were not developed for all equipment, such as, but not limited to, Press 66, Assembly Machines 148, 149, 153, 476, 478. b. Lockout/tagout specific procedures were not developed for machines/equipment associated with the injection molding machines, such as, but not limited to, temperature controls units (such as TC266 at injection molding machine C31) and assembly machines (such as 246 at injection molding machine F20). Abatement documentation is required for this violation.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a. Keyed switches, not allowed as lockout points according to the standard (it was also stated in the employer's lockout program - Definitions 3.5) were indicated in the employer's lockout specific procedures as a lockout point, such as on injection molding machine C31 for the hydraulic motor (E2) and the robot (E3). (Note: OSHA INSTRUCTION CPL 02-00-0147 states that electronic devices are not acceptable lockout points.) b. Not all lockout points were listed for lockout for machine equipment, such as, but not limited to, the circuit breaker energy isolation device for robots, such as for injection molding machine C31. Abatement documentation is required for this violation.
