29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition; drainage was not maintained:   In building 5, the below grade scrap conveyor floor was covered with cutting oil, resulting in a slipping hazard.  Only one sump pump was installed, which was insufficient to remove fluid from the pit walking/working surface.  Abatement documentation is required for this violation.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  In building 5, the below grade scrap conveyor work area, which was littered with metal scrap and covered in fluid which at times has been approximately one inch or more deep, was not assessed for hazards requiring the use of personal protective equipment such as boots impermeable to the water soluble cutting fluid.  Abatement documentation is required for this violation.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:  Lockout devices were not available for all of the types of energy isolation devices such as for pneumatic energy sources, gravity, and for large type breaker switches.  Examples were provided of work requiring energy isolation of pneumatic energy sources.  No pneumatic lockout devices were available in the building 5 maintenance shop.  Hinged tables were isolated from falling by a device that was not locking in some manner and could easily be kicked out.  Abatement documentation is required for this violation.

Employees were exposed to the potential energization of the conveyor during work on the conveyor, as well as potential overhead struck by hazards as a result of operation of the stamping presses during work in the conveyor pit.
