OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to vehicle struck by and crush hazards.     a) Jobsite: Finish area - On June 27, 2016, a vehicle was conveyed through the car wash tunnel and came to rest approximately 8 feet beyond the tunnel after exiting. An employee inside the vehicle remained to clean the interior windows as other employees began cleaning the vehicle's exterior. As one employee was cleaning the exterior rear window, the vehicle began to roll backwards, then accelerated and went into the tunnel, where it struck a second vehicle and crushed the employee, who suffered fatal chest injuries.

A description of how the criteria in paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets, and employee information and training will be met. 2.   A list of hazardous chemicals known to be present in the workplace. 3. The methods the employer will use to inform employees of the hazards of non-routine tasks.
