29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Warehouse Area - On or about March 10, 2016, an employee was exposed to fall hazards due to working on a Safety Cage attachment lifted up approximately 26 feet in height from the ground using the Komatsu forklift (Model: FG25T-16, Serial # 216482A) operated by a host employer employee without having the manufacturer's written approval of the use of this attachment. b) Warehouse Area - On or about May 2, 2016, an employee was exposed to fall hazards due to working on a Safety Cage attachment lifted up approximately 26 feet in height from the ground using the Komatsu forklift (Model: FG25T-16, Serial # 216482A) operated by a host employer employee without having the manufacturer's written approval of the use of this attachment.
