OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, in that employees were exposed to a caught-in hazard: On or about June 21, 2025, in the Reel and Winder Section of building #8 on a 22- ton bridge crane, employees were exposed to caught-in hazards while performing maintenance work activities including, but not limited to hand-over-hand spooling of wire rope onto or off the drum, making hand adjustments to equipment, removing or adjusting wire rope clamps, and cleaning and lubricating the drum, while not maintaining control of the crane control mechanism and while using a handheld mobile device.
