29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.     a) At the jobsite, on or about July 1, 2016, the operator dismounted the JLG Telehandler rough terrain forklift Model: 642, S/N: 0160074153 with an occupied personnel work platform (a pallet) that was elevated, which exposed an employee to fall hazards up to 19 feet onto the concrete floor (inside the structure) or ground (outside the structure).

29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:    a) At the jobsite, on or about July 1, 2016, an employee performing steel erection activity was elevated and working from an unapproved work platform (a pallet) that was supported by a JLG Telehandler rough terrain forklift Model: 642, S/N: 0160074153, which exposed the employee to fall hazards up to 19 feet onto the concrete floor (inside the structure) or ground (outside the structure).     Ronald Weeks dba RTW Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(a)(6), which was contained in OSHA inspection number 899127, citation number 1, item number 004a and was affirmed as a final order on May 5, 2014, with respect to a workplace located at 5 Chandler Street Lot # 14 Richmond Hill, Georgia.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a) At the jobsite, on or about July 1, 2016, an employee performing steel erection activity was elevated and working from an unapproved work platform that was supported by a JLG Telehandler rough terrain forklift Model: 642, S/N: 0160074153.  The unapproved work platform was not secured to the forks or carriage and was moved horizontally to reposition the employee, which exposed the employee to fall hazards up to 19 feet onto the concrete floor (inside the structure) or ground (outside the structure).      Ronald Weeks dba RTW Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(c)(2)(v), which was contained in OSHA inspection number 899127, citation number 1, item number 004b and was affirmed as a final order on May 5, 2014, with respect to a workplace located at 5 Chandler Street Lot # 14 Richmond Hill, Georgia.
