29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems or safety nets. (a) On July 7, 2016, at the beach front pavilion, two workers were renovating/removing damaged roof tiles 10 feet from the ground level without fall protection systems such as a full body harness or guardrail system, exposing workers to fall related injuries.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: (a) On July 7, 2016, at the beach front pavilion, the ladder used to access the roof was not placed at an angle that was approximately 75 degrees from the horizontal surface of the ground exposing workers to a fall hazard.
