29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about July 8, 2016 electricians installing cove lighting were working on energized 277 volt circuits.  This exposed electricians to the hazard of electrical shock at the construction site.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit:  On or about July 8, 2016 the employer did not ensure that electrical circuits were de-energized and tested before the cove lighting using the circuits was installed, exposing employees to the hazards of electrical shock.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately:  On or about July 8, 2016 and in times prior thereto, a 16 foot fiberglass step ladder being used by electricians to install cove lighting had cracks in the side rail, exposing employees to the hazards of falls up to 16 feet.
