Note: The requirements applicable to construction work under this paragraph are identical to those set forth at '1910.178(l) of this chapter.    29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) 31 Sheller Drive, Monroe Township, NJ 08831:  The employer did not ensure that the operator of the Square Shooter SS-636 rough terrain forklift had been adequately trained.    This hazard was observed on 7/9/16.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) 31 Sheller Drive, Monroe Township, NJ 08831:    On 7/9/16 employees were exposed to fall hazards of up to 23 feet while performing residential framing activities without an adequate fall protection system in place.    b) 31 Sheller Drive, Monroe Township, NJ 08831:    On 7/11/16 employees were exposed to fall hazards of up to 14 feet 7 inches while performing residential framing activities without an adequate fall protection system in place.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) 31 Sheller Drive, Monroe Townhip, NJ 08831:  The employer did not ensure, throught a wrritten certification, that employees had been adequately trained in the hazards associated with falls and the proper ways and procedures to take in order to minimize the hazards.  This hazard was observed on 7/9/16.
