Note: The requirements applicable to construction work under this paragraph are identical to those set forth at '1910.178(l) of this chapter.  29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l).  The certification did not include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training and evaluation.  A) 1 Queens Ct, Monroe Township, NJ 08831  The employer did not ensure that the operator of the Terex SS842 rough terrain forklift had completed refresher training and had the proper documentation of the training.  Occurred on or about 10/06/21  Celso X. Chay was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.602(d) , which was contained in OSHA inspection number 1330733, citation number 2, item number 1 and was affirmed as a final order on 08/21/18, with respect to a workplace located at 5 Washington Crossing, Monroe Township, NJ 08831
