On June 28, 2016, at the quarter pound filler operator area, there were no guards on two horizontal rotating shafts 32 inches from the from the floor and projecting more than 1/2 of shaft's diameter, exposing the operator to caught-in hazards. b. On June 28, 2016, at the Fill and Cap area, there were no guards on two horizontal rotating shafts 40 inches from the from the floor and projecting more than 1/2 of shaft's diameter, exposing the operator to caught-in hazards.
