29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards.      On the south west side of the storage warehouse, an employee was exposed to a fall hazard of approximately 12 feet to the concrete floor without fall protection while working on the top level of a storage rack.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.       On the south wall of the storage warehouse, the employer did not ensure employees had clear access to the fire extinguisher exposing them to fire and smoke hazards.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.   On the south west side of the storage warehouse, an employee suffered multiple injuries from a fall of approximately 12 feet from a storage rack.  The employer did not report the accident to the Occupational Safety and Health Administration within 24 hours.
