29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) In the main shop area, the point of operation of a 60-ton Niagara  press brake, model number 18-36-6-8, serial number 44-741, was not guarded, exposing employees to amputation hazards.  b) In the main shop area, the point of operation of a 140-ton WYSONG  press brake, model number FAB140-168, serial number HPF27-100, was not guarded, exposing employees to amputation hazards.   c) In the main shop area, the point of operation of a 75-ton Chicago press brake, model number 810-C, serial number L-10052, was not guarded, exposing employees to amputation hazards.   d) In the main shop area, the point of operation of a 160-ton Durma press brake, model number E 30160, serial number 7541072963-E, was not guarded, exposing employees to amputation hazards.   e) In the welding area, the point of operation of a RAMCO horizontal band saw was not guarded, exposing employees to amputation hazards.  QuietAire Corporation was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 116858, citation number 1, item number 2 and was affirmed as a final order on June 9, 2016, with respect to a workplace located at 505 N. Hutcheson St. Houston, TX 77003.
