29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: (a) Evisceration Line 1 - On or about July 14, 2016, there were three (3) unguarded openings in the grate over the floor drain trough into which the employee cleaning the Cropper machine could accidentally walk. (b) Chemical storage - On or about July 14, 2016, there was an unguarded hole near the middle of the floor into which employees could accidentally walk.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a) DAPEC line or free hang - On or about August 16, 2016, the two elevated platforms were missing a top rail leaving a midrail at 20 inches, some railings had a top rail at 29 inches. (b) IQF room - On or about August 16, 2016, the two elevated platforms at the Z belt conveyors did not have a mid rail on one of the sides.
