29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:   On or about 07/06/16, at the job site, a flexible cord that was connected to a Makita circular saw used to cut 2"x4" was spliced in several locations of the cord. 29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  On or about 07/06/16, at the job site, a Genie telescopic forklift equipped with an unapproved non attached work platform was used to lift personnel approximately 15 feet from ground level. 29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  On or about 07/06/16, at the job site, employees were exposed to a fall hazard of approximately 15 feet to the unpaved ground level below while working from a scaffold that was not equipped with a guardrail system on all open sides, where employees wearing a fall arrest system while installing rebar from a scaffold platform and not attached to an anchorage point, and where employee access to the scaffold was by a Genie telescopic forklift equipped with an unapproved non attached work platform.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
