29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter:   a)    East Coast Barge & Boat Company, Inc. On or about 07/13/2016, several welders were not provided with an annual respirator fit test since May and October 2015. Welders wore a 3M 6000 series half face tight fitting respirator to weld and carbon arc cut on carbon and alloys mixed steel without being annually fit tested.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI):    a)    East Coast Barge & Boat, New Construction: On or about 07/13/2016, the employer did not make an initial determination for employee exposure to chromium (VI), where the SDS for Ryerson Carbon and Alloy Steels shows chromium up to 11 percent and a hazard identification states welding may generate airborne concentrations of hexavalent chromium, and where employees performed welding tasks on the steel, exposing welders and shop workers to an increased risk of respiratory irritation, sensitization or asthma, skin sensitization or allergic contact dermatitis, and eye irritation.    Note: The employer is to make this determination in accordance with either paragraph (d)(2), the scheduled monitoring option, or paragraph (d)(3), the performance-oriented option.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement

29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches.      a)    East Coast Barge & Boat, new barge construction: On or about 07/14/2016, a cover or a suitable guard to a height of not less than 30 inches was not provided to protect an employee sweeping near an 18-inch flush manhole topside of a barge as a result an employee was exposed to an 8 feet fall hazard.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
