29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about July 19, 2016, an employee using a Hitachi NR83A3 pneumatic nail gun to conduct framing operations was not wearing safety glasses, exposing himself to being hit by nails or flying debris.

On or about July 19, 2016, on a 6:12 pitched structure, an employee performing framing operations was not protected from an 11-foot fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.
