29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about May 16, 2017, the Werner 24-foot extension ladder was not extended 3 feet above the landing and was not secured to prevent accidental displacement.  Employees were exposed to a 9 feet fall hazard.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a. On May 16, 2017, an employee using a nail gun to install decking was not wearing safety glasses, exposing himself to being struck by flying debris or nails.     ASG Framing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1162915, citation number 01, item number 002 and was affirmed as a final order on August 22, 2016, with respect to a workplace located at 5199 Holly Fern Trace, Tallahassee, FL 32312.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On or about May 16, 2017, roof area, two employees installing decking on a 7:12 pitch roof were not protected from up to a 23 feet fall hazard by the use of a fall protection system.     ASG Framing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1162915, citation number 01, item number 003 and was affirmed as a final order on August 22, 2016, with respect to a workplace located at 5199 Holly Fern Trace, Tallahassee, FL 32312.
