29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 11 July 2016, at that above addressed jobsite, four employees were installing plywood on the roof of the apartment complex under construction, without the use of fall protection, exposing the employees to a fall hazard of approximately 18 feet in height to the ground below.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge: On or about July 11, 2016 at the above addressed jobsite employees were accessing the second floor of an apartment complex under construction by utilizing a cement stairway that had unprotected sides and no stair rail system, exposing the employees to a fall hazard of approximately 10 feet and 8 inches.
