29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Meat cutting room, Save-a-Lot, 459 Main Street, Indian Orchard, Massachusetts; On July 14, 2016 and times prior, the employer required the meat cutters to adjust the upper guide and guard assembly on the Hobart bandsaw to 1/2 inch above the piece being cut, exposing employees to an amputation hazard.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Warehouse, Save-a-Lot, 459 Main Street, Indian Orchard, Massachusetts; On July 20, 2016 and times prior, the employees charging the electric pallet jack in the warehouse were exposed to 120 volt electrical shock hazards when using an extension cord that was missing the ground pin.
