OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that an employee was exposed to an amputation hazard while working with a log splitter.      a)  Back Yard: An employee was exposed to an amputation hazard while using a log splitter. On 3/18/2016, an employee sustained an amputation to the index finger and a severe laceration to the middle finger on the right hand while assisting with the loading logs into a Timber Wolf log splitter.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:      a)  Back Yard: The employer failed to report to the Occupational Safety and Health Administration within twenty-four (24) hours of an incident in which his employee sustained an amputation to the index finger and a severe laceration to the middle finger on the right hand while assisting with the loading logs into a Timber Wolf log splitter on 3/18/2016.
