29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:     a)  On or about July 20, 2016, a "Dyne" unit being unloaded from a delivery truck by a Rockwell Automation, Inc. employee, using a Toyota electric lift, allowed the delivery truck driver to be in direct contact with the unstable ("Dyne" unit on a pallet) load while the load was being moved.  This exposed the truck driver to struck-by and crushing hazards.
